You're missing the glaringly obvious.
Your body is only as good as you keep it. Drugs and going balls to the wall on heavy workouts several days a week, sometimes twice a day for months and years takes its toll.
You abuse alcohol, you liver, kidneys, skin and overall organ health will deteriorate. Same goes for those who give up everything to get that perfect physique.
Everything you put into your body the stomach, intestines, heart, liver and kidneys need to work. Lots of guys have died through kidney problems and I have done extensive research by myself and alongside Doctors because I have a very real kidney problem.
Once they start to go your shelf life is reduced to months or 2 years and remember they do not have regenerative qualitives like the liver. I'm not sure why so many guys at the top of their game ignore this but its their loss.
I haven't even mentioned the problems with joints and tendons.

Some cultures appear to me to have a different relationship to athletics and fitness than I was raised with (west coast white American middle class). Instead of believing in some level of lifelong fitness for everyone, there is a strict binary: you are either an athlete or you are not. If you aren't then you don't do anything.
I'm thinking about the latino/Central American immigrant community, and the working-class African American community, especially the women. You'll see jacked, athletic black guys while their female counterparts are obese and soft. Latin guys may not be jacked but they stay trim--maybe work, soccer, and I used to see quite a few at the gym. The young latina moms turn into potato sacks. In each case the binary seems to play out as "sport/fitness is a men's thing."
Obviously with the examples in this thread we aren't talking about the same gender dynamic, but some of the examples at least are also from different cultures and I just wonder if a similarly stark binary is at work. I'd be interested to hear comments from guys in or with a better understanding of other cultures' attitudes because I am very much speculating from the outside.

Better to develop a consistent habit that lasts overtime.
Push yourself in the gym, but don't figuratively kill yourself in there.
3-4 times a week is enough.
Play sports or swim to keep your body guessing and your fitness in good shape.
Get lean, put on muscle, and stay that way.

As far as the body builders go, it's largely artificial and the body cannot handle the abuse required to maintain that body (drugs/wear and tear) long term. It like anything else, you drive with your foot to the floor all the time, something is going to blow. There are a lot of natural bodybuilders well into their 60's that still look good, moderation is key, which is why I switched to full body workouts like rowing and cycling/MTB 15 years ago ( I'm 60).

I can't really judge Naseem, he did go through a horrible car accident that probably leaves him unable to workout.
Injuries suck ass, and can happen to anyone. No need to hate. Even still, his wife looks like she cannot handle it. Sucks for her. She leaves him and feels like a dirtbag, but if she stays she's stuck with a fat goofball.
I don't think you can get that fat unless your eating gets out of hand. What you eat matters a lot more than how much you exercise as far as getting fat.
He's short, so that makes putting on the pounds easier too.

There have been days where I couldn't be arsed going to the gym after work or weekends and I just ate shit all day. The lifestyle athletes lead would be depressing. For girls you cannot even go on birth control or have much of a sex life and the guys need to be at the gym early, have their diets keyed in by nutritionist and if they deviate they can lose their spot.
Imagine going for a pub meal, seeing all the delicious meals but knowing you cannot have that delicious meat dish or a nice pasta based meal. Fuck that.
Athletes are always training to get those times and numbers all the time and doubly-so when competition is a few months off. No wonder the athletes village is an outright fuckfest at every competition.

I used to live next door to an NHL player. One evening the neighbors got together and he came over. We were eating snacks (nuts) and drinking beers. He drank one beer and are some of the snacks, then said he better stop, he had a game the next day. He said he was going to have a big pasta meal. Not that relevant , but interesting to see the inside life of a professional athlete.
I saw him once during the off season. He said he was bored out of his fucking mind, couldn't wait for the season to start. He said it's hard to not sleep til noon ever day. Oh, he was taking fighting lessons during the summer. Some boxer was giving him lessons, and he was watching hockey fight videos. He said its key to pull the other guys jersey halfway off so he can't punch back.
