Don't use spermicide. It doesn't decrease failure rates significantly and comes with other problems - shorter shelf life, allergies. If you're really paranoid, don't come inside them. I've used all kinds of condoms for 15 years now, I've never had one break. The only chance of a condom breaking if you wear it in reverse.
