I'm looking forward to an iOS app in the future (probably a generation away) called To Fuck or not to Fuck which can do full STD testing as some home testing kits can do today and can give the percentage chance of her becoming pregnant. Truth be told OP, pretty much the only full proof way to avoid pregnancy is to not have intercourse:

Condoms can break

Pulling out can still impregnate the woman from 'pre cum'

Women can forget to take the pill (or even lie about it) and female birth control has a chance of not working

I've even heard stories of guys having vasectomies and still ending up getting a woman pregnant.

Other than a hysterectomy or exclusively banging MILFs who are past menopause there's no way to guarantee it won't happen, only reduce the odds.

So anytime you put the penis in the vagina you're taking that risk - if you're 100% sure you aren't in a position to have a kid then like it or not your safest option is to avoid intercourse entirely, or stick to foreplay, oral sex, etc.

Other than that most girls you bang will eventually just tell you they are on birth control at some point even if you didn't ask.

I used to break conforms a lot until i realized that the fit and lubrication was not optimal. All condoms and all penises are not made the same.
