You'd be surprised. The medical technology for this stuff has come a long way since the bearded ladies I used to see walking down Santa Monica blvd. They are also getting started at a much younger age. It's much easier to "pass" when you've never fully went through puberty as a male first. Those changes are harder to reverse once they've occurred. Some of these people are starting at age 15. They are also very skilled at hiding things during sex, even if they haven't had the "operation."
