I think it's mostly emotional eating. There's a huge connection between obesity and childhood abuse but nobody seems to want to talk about it. The modern diet might play some role by making the consequences of overeating worse but the drive is almost certainly psychological. I've heard people who've lost massive amounts of weight talk about how it made them feel defenseless and vulnerable. The fat was a suit of armor for them keeping other people at arms length (physically and emotionally).
