I'm fairly sure that the level of transgenderism in society would be very proportional to the acceptance of polygamy, overt or covert.
The modern western world has probably the highest rate of polygamy in the history of the world. Famous men, players, rich guys, have access to hundreds if not thousands of women in a lifetime. More than any emperor save Ghengis Khan perhaps.
Transgenders have historically been accepted in societies with huge differences in wealth and power, South East Asia, Iran/Afghanistan, India etc.
